Telephone Consultations
Telephone consultations with the doctor can be arranged by appointment at certain times during the day.
Home Visits
If you are too ill to come to the surgery, you can request a home visit. Please telephone as early as possible, before 10am, and leave your phone number in case the doctor wishes to speak to you. It is helpful if you give the receptionist some idea of the problem so that the doctor can decide how quickly to visit.
If your visit is urgent, your own doctor may not be available but one of the partners will be on duty and will be able to attend to you.
Out of Hours
The Collingham practice is part of West Yorkshire Urgent Care Services which provides out of hours care for their patients.
If you have need of urgent medical attention when the practice is closed, please phone the surgery and listen to the recorded directions; if required the telephone system will transfer your call to West Yorkshire Urgent Care Services. Should you only require medical advice, you will be given the number for NHS Direct.
The out of hours service is available for Collingham patients from 18:00 Friday to 08:00 Monday and then 18:00 to 08:00 Monday to Thursday. The service is also available between 13:00 and 14:00hrs for patients who ring the Collingham surgery number where there are recorded instructions for the patient to follow. If required, the phone system will transfer you to the out of hours service provider.
The out of hours service is available for Thorner between 18:00 Friday to 08:00 hrs Monday and then 18:00 to 08:00 Monday to Thursday. There are recorded instructions for the patient to follow. If required, the phone system will transfer the patient to the out of hours service provider. Outside of these hours, please ring the surgery at Collingham for urgent medical attention.
Repeat Prescriptions
Prescriptions of medicines taken regularly can be obtained without seeing your doctor provided that the doctor has agreed to this. All repeat prescriptions are held on computer. Please post or deliver your repeat prescription computer slip to the surgery. They also can be faxed on 01937 574754. For all repeats please allow 48 hours before collecting. Dispensing patients may expect to collect their medicines ready prepared.
Results of Blood Samples, Tests etc.
Please telephone for these in the afternoon. Only available to the patient, not to patient’s representative. (Exception – results for children).
Training of Medical Students
Occasionally we have students in the practice who are training to be doctors. This will be mentioned to you in advance and if you prefer to see the doctor or nurse alone please tell the receptionist.
Disabled Patients
All our surgeries are designed with access for the disabled and infirm: We have easy wheelchair access to the building and consultation rooms and there are toilet facilities for wheelchair users. We also have an induction loop system for the deaf and hard of hearing.
