At Collingham Church View Surgery we are committed to providing a high standard of care to all our patients. However, we recognise that there may be occasions when our service does not meet your expectations.
If you have a complaint or concern about the care or service you have received from any of our doctors or staff, we encourage you to let us know. We operate a complaints procedure in line with NHS requirements and national standards for handling patient feedback and complaints.

We hope that most concerns can be resolved quickly and informally, often at the time they arise and directly with the person involved. If this is not possible, or if you wish to make a formal complaint, please contact us as soon as you can.
To enable us to investigate your concerns effectively, complaints should normally be made:
- Within 6 months of the incident giving rise to the complaint; or
- Within 6 months of becoming aware of the issue, provided this is no more than 12 months after the incident occurred.
Complaints should be addressed to the Practice Manager, Eliza Venizelou, or to any of the Doctors at the practice. Please note that your complaint may be investigated and responded to by another member of the management team where appropriate.
Alternatively, you may request an appointment to discuss your concerns in person. We will explain the complaints process and make every effort to address your concerns promptly, fairly, and sensitively.
To help us investigate your complaint thoroughly, please provide as much specific information as possible about the issue you wish to raise.
